Recurrent collapse of the upper airway during sleep causes repetitive episodes of intermittent hypoxia, and recurrent arousals from sleep, leading to daytime sleepiness, poor quality of life, and increased risk of road traffic accidents, cardiovascular morbidity and mortality, and metabolic dysfunction.Hereditary basis:OSAS is approximately twice as common in males. Up to 40% of the variability in sleep breathing can be accounted for by heritable factors. However, a discrete genetic basis is unlikely in the majority of cases.Environmental factors:Obesity is the major reversible risk factor for the development of OSAS. Factors such as alcohol use and sedative medications may contribute to disease severity.Differential diagnosis:Simple snoring, obesity hypoventilation syndrome, periodic limb movements of sleep, narcolepsy, shift work sleep disorder, congenital central hypoventilation syndrome (Ondine’s curse).
